How Much Does a TMS Machine Cost? (2026)

A new TMS machine typically costs between about $50,000 and $200,000, depending on the brand, the coil and what is bundled with it. Used systems are commonly listed at roughly $35,000 to $120,000. The purchase price is only part of the cost, though: annual service, consumables and, on some systems, per-treatment fees can add tens of thousands of dollars over five years.

The ranges below come from published sources, each dated and linked. They are not HealthTech USA quotes. For a quote on an Apollo TMS system with installation, training and service, contact our team.

Two more reference points. Neuronetics' audited 2025 results imply an average of about $88,800 per NeuroStar system shipped (2025 system revenue divided by systems shipped, Neuronetics). And a 2021 comparison chart from The Carlat Psychiatry Report listed prices from about $53,000 to $220,000 across eight FDA-cleared systems. Treat aggregator ranges as estimates; actual pricing depends on the configuration and the deal.

The costs after the purchase

When we talk with practice owners, price and return on investment are the first questions. The answer depends on the costs that are easy to miss:

  • Per-treatment fees. Some systems charge for each treatment after the purchase. NeuroStar's US treatment-session revenue was $43.3 million in 2025 (Neuronetics), which shows how much of that model's cost comes after the sale.
  • Service and warranty. Published estimates run about $4,500 to $10,000 a year for major brands.
  • Consumables. Treatment caps and similar items run an estimated $1,500 to $5,000 a year on some systems.
  • Coil replacement. Replacement coils can cost $5,000 to $20,000 each on some used systems (Device Pulse).
  • Software transfer fees on used machines. Re-registering software on a used system can cost several thousand dollars or more.
  • Training, staffing and downtime. These do not show up on an invoice, but they decide how many sessions you deliver.

"Beware of the hidden costs most manufacturers do not disclose."

A simple five-year cost worksheet

Add up these lines for each system you are comparing:

  1. Purchase price, or the total of lease or financing payments
  2. Installation and training
  3. Five years of service contracts or warranty
  4. Five years of consumables
  5. Per-treatment fees × the sessions you expect to deliver
  6. Expected parts replacement, such as coils

Illustrative example only: a $90,000 system with $7,000 a year of service and $3,000 a year of consumables comes to $140,000 over five years before any per-treatment fees. Run the same worksheet with your own quotes; the cheapest purchase price is not always the cheapest system to own.

How reimbursement fits in

TMS is billed with CPT codes 90867 (initial treatment, including cortical mapping and motor threshold), 90868 (subsequent treatment delivery and management) and 90869 (motor threshold re-determination with delivery). For 2026, Medicare does not publish a national payment amount for these codes. They are contractor-priced, so each Medicare Administrative Contractor sets the rate (CMS RVU26D). Commercial payers set their own rates and coverage criteria, so check your top payers before you model revenue.

On the tax side, the 2026 Section 179 expensing limit is $2,560,000, phasing out above $4,090,000 of qualifying equipment (IRS Rev. Proc. 2025-32). Ask your CPA how it applies to your practice; this is not tax advice.
